You may schedule an appointment with your primary care physician by calling his or her office during normal business hours. Your physician's medical receptionist will assist you in scheduling an appointment to accommodate your schedule.

When visiting your doctor's office, bring your health insurance card and co-payment, if applicable. If you are new to our Group, please allow approximately 30 extra minutes before your first scheduled appointment to complete paperwork and give us a chance to become acquainted. We encourage you to make an appointment with your doctor as soon as possible to determine your personal healthcare needs and coordinate any ongoing care.
