Our Santa Ana Medical Facility offers an on-site, self-service Health Resource Center that provides patients with:

• Information on health and wellness topics relevant to adults, parents and teenagers.
• Forms and applications for Advance Directives, Medi-Cal, transportation, utility assistance and more.
• Computer access enabling individuals to print health information, watch surgical videos and participate in tutorials and health risk assessments.
• Brochures in Spanish and other languages, which can also be found online by clicking here. You may also visit the St. Joseph Hospital Resource Center or Burlew Medical Library for additional information.
