The Center for Health Promotion offers a care coordinator to assist patients in accessing the resources they need to maintain their health and well being. The coordinator provides:

• Emotional support to patients and loved ones when adjusting to a medical condition
• Referrals to behavioral health services, counseling and psychotherapy services

The care coordinator also provide information and referrals for:

• Food
• Clothing
• Housing
• Prescription assistance
• Parenting classes
• Substance abuse treatment
• Financial assistance
• Transportation
• Legal assistance
