St. Joseph Heritage Healthcare offers the patients of St. Joseph Hospital Affiliated Physicians (SJHAP) and St. Joseph Heritage Medical Group (SJHMG) a wide variety of group and individual educational programs to help you and your family improve and maintain a healthy lifestyle.  These programs are made possible through the funding and support of St. Joseph Hospital, St. Joseph Hospital Affiliated Physicians, and St. Joseph Heritage Medical Group.

Pre-registration is required for all programs.

Group Programs

Live Well, Live Healthy
Our two-session program provides the latest guidelines for those concerned about high cholesterol, high blood pressure, pre-diabetes, weight problems, or for those interested in adopting a healthier lifestyle.  This program is offered in a class setting at convenient times throughout the week.

Diabetes Awareness
Our four-session diabetes program is designed to help those newly diagnosed with Type 2 diabetes or pre-diabetes, or those just wishing to brush up on diabetes management skills.  A free blood glucose meter will be provided in class. This program is offered in a class setting in morning, afternoon, and evening sessions. Classes available in Spanish.

Diabetes and Pregnancy Program (Sweet Success)
This highly regarded program is for women with diabetes who are pregnant, including those with gestational diabetes.  Women referred to this program will receive regular, individual counseling during and after their pregnancy.  Your doctor's referral is needed to enroll in this program and you must be delivering at St. Joseph's Hospital in Orange. Program available in Spanish.

Individual Consultations

Dietitian Consultations
We offer individual counseling with a Registered Dietitian based on patient diagnosis, to help develop nutritional strategies and lifestyle changes for better health.  Your doctor's referral is required.

Pediatric Weight Management
We offer individual counseling with a Registered Dietitian for children and their parents to develop healthy eating habits for the entire family.  Your doctor's referral is required.

Asthma Education
A health educator will work with you to develop strategies and tools that will help you breathe easy and stay in control.

Support Groups

Diabetes Support Group
Our ongoing support group is designed to provide a welcoming, non-judgmental environment for patients living with diabetes.  This group allows individuals to share their thoughts and feelings with other patients living with diabetes. For more information call (714) 628-3245.
